We accept most major insurance company plans, including: Blue Cross/Blue Shield, Arnett, HMO, City of Lafayette, Caterpillar, Purdue, Alcoa, and Care Source.
For those without insurance, we offer an option of cash, personal check, VISA, or MasterCard.
